Data Privacy @ VERSO: Recording ATK Results

Statement on Data Privacy and Data Protection regarding your COVID-19 Antigen Test Kit (ATK) Results:

The COVID-19 ATK Results Form requests for your Medical Data to be shared with VERSO through a Google Form from which a link brought you to this page.

What information are we collecting from you?

• First and Last Name

• VERSO Email Address

• VERSO Department

• COVID-19 ATK Result

Why are we collecting your data?

• Your name and contact details are necessary to allow us to properly record your COVID-19 ATK result.

• Given the current condition of COVID-19 in Thailand, we are following government regulation that requires us to collect regular COVID-19 ATK results from employees, students, parents, and visitors entering our campus.

• This data will allow our school to comply with government regulations.

How will we use your data?

• We will only use your data internally at VERSO. We are not sharing your ATK results with any 3rd party organization.

What is the legal basis for processing the data?

• We are collecting ATK results data to support the health and safety of employees, students, parents, and visitors at VERSO International School.

• We process your health data (personal sensitive data) under your consent.

If we are sharing your data with others, who are we sharing it with?

• Your data is only viewable and accessed by a small Task Force composed of five members coming from VERSO's HR, Communications, Security, and Technology Departments and the School Nurse. As needed, the Head of School may also be informed about your COVID-19 ATK results.

• As needed, we will share the student's medical history information with a hospital or doctor in the event of an emergency.

• Your data is kept in this Google Form as well as the related Google Spreadsheet.

How long will we process your data for?

• Data you enter in this form will be kept with your employee record for as long as required by the Thai government for compliance purposes. When you leave VERSO, the data is kept and eventually deleted following our data retention guidelines.

What are your rights and how can they be enforced?

• Right to be informed

• Right of access

• Right to rectification

• Right to restriction

• Right to erasure (health data)

• ผลการตวจหาเชื้อโควิดโดย ATK

สาเหตุที่่เรจึงรวบรวมข้อมูลของคุณ

• ชื่อและรายละเอียดการติดต่อของคุณมีความจำเป็นในการบันทึกผลการตรวจหาเชื้อโดย ATKได้อย่างถูกต้อง

• เนื่องจากสถานการณ์ปัจจุบันของโควิด19 ในประเทศไทย ทางโรงเรียนได้ปฎิบัติตามข้อกำหนดของทางรัฐบาลซึ่งจะต้องมีการรวบรวมผลการตรวจหาเชื้อโควิดโดย ATK จากพนักงานทุกคน นักเรียน ผู้ปกครองและบุคคลภายนอกที่เข้ามาเยี่ยมชมโรงเรียน

• ข้อมูลนี้จะช่วยให้ทางโรงเรียนสามารถปฏิบัติตามข้อกำหนดของทางรัฐบาลได้

เราจะใช้ข้อมูลของคุณอย่างไร

• เราจะใช้ข้อมูลของคุณภายในโรงเรียนนานาชาติเวอร์โซเท่านั้น เราจะไม่เปิดเผยผลการตรวจหาเชื้อโควิดโดย ATK ของท่านแก่บุคคลที่สาม

อะไรคือการประมวลผลข้อมูลขั้นพื้นฐาน

• เรารวบรวมผลการตรวจหาเชื้อโควิดโดย ATK เพื่อสร้างความปลอดภัยให้แก่พนักงานที่โรงเรียนนานาชาติเวอร์โซ

• เราดำเนินการใช้ผลข้อมูลสุขภาพของคุณ (ข้อมูลส่วนบุคคลที่ละเอียดอ่อน) ภายใต้ความยินยอมของคุณเท่านั้น

หากเราเปิดเผยข้อมูลของคุณกับผู้อื่น เราจะเปิดเผยกับใครบ้าง

• ข้อมูลของคุณสามารถดูและเข้าถึงได้โดยฝ่ายทรัพยากรบุคคล ฝ่ายพยาบาล ฝ่ายการตลาด ฝ่ายรักษาความปลอดภัยและฝ่ายเทคโนโนลีของโรงเรียนเท่านั้น และจะมีความจำเป็นจะต้องการแจ้งผลไปยังครูใหญ่ของโรงเรียนด้วย

• มีความจำเป็นที่จะต้องเปิดเผยข้อมูลประวัติการรักษาของนักเรียนให้แก่โรงพยาบาลหรือแพทย์ในกรณีฉุกเฉิน

• ข้อมูลของคุณจะถูกเก็บไว้ในแบบสอบถามนี้เช่นเดียวกับเอกสารข้อมูลที่เกี่ยวข้อง

เราจะดำเนินการใช้ข้อมูลของคุณนานแค่ไหน?

• ข้อมูลที่คุณกรอกในแบบสอบถามนี้จะถูกเก็บไว้กับบันทึกพนักงานของคุณตามข้อกำหนดของรัฐบาล และจะถูกลบออกเมื่อคุณออกจากโรงเรียนนานาชาติเวอร์โซ

คุณมีสิทธิอะไรบ้างและจะบังคับใช้ได้อย่างไร?
